Eric Olsen is the Chief Executive Officer of E.O. Construction, doing business as an Employer at 305 W. Gaviota, San Clemente, CA (work site). On September 9, 2005, one of the Employer's employees was injured when he fell at the work site. On September 30, 2005, Dennis Nagel, Associate Safety Engineer for the Division of Occupational Safety and Health (Division), commenced an accident inspection at the work site where the employee was injured. On October 21, 2005, the Division issued citations as set forth below.1
