On April 22, 2005, the Division of Occupational Safety and Health (the Division), conducted an investigation at a place of employment maintained by Employer at 655 West Broadway, San Diego, CA. On July 14, 2005, the Division, through Kimlee Lindgren, Associate Safety Engineer, cited Employer for the alleged violations of the occupational safety and health standards and orders found in Title 8, California Code of Regualtions.¹
