Employer provides lawn and tree lanscaping services. On February 8, 2004, the Division of Occupational Safety and Health (the Division), through Associate Safety Engineer Dennis Barker, conducted an accident investigation at a place of employment maintained by Employer at 2355 Country Road So., Willows, California (the site). On August 4, 2004, the Division cited Employer for the following alleged violations of the occupational safety and health standards and orders fount in Title 8, California Code of Regulations.
