Between October 21, 2005, and March 17, 2006, the Division of Occupational Safety and Health (the Division), through Associate Safety Engineer Dennis Barker, conducted and accident investigation at a place of employment maintained by Employer at Market Street, and Riverside Drive, Redding, California (the site).
